The real reason

## Why your hip keeps hurting

Your hips are meant to drive and absorb load. When they can't, something else takes over, and the hip ends up irritated anyway.

### Movement got stuck

If the hip can't rotate or extend well, every squat, stride and swing jams it into the same range, over and over.

### The control isn't there

When the pelvis and trunk don't control the leg, the joint and the tendons around it take the hit. An old injury on one side is often where it started.

### Stretching doesn't fix it

Tight hips are often guarding. Stretching feels good for an hour. Building strength and control through the full range is what changes it.

## Why most hip rehab falls short

Clamshells, a foam roller and a stretching routine. Most hip rehab treats the symptom and never asks why the hip got overloaded in the first place.

An MRI finding isn't the whole story either. Labral tears and impingement show up in plenty of people with no pain at all. What matters is how your hip handles load, and that can be trained. We keep going until you're squatting, running or playing again without thinking about your hip.

## Frequently asked questions

### Can physical therapy help a hip labral tear or impingement without surgery?

Often, yes. Labral tears and impingement show up on MRIs in many people with no pain. Building hip strength, control and range often settles symptoms without surgery. If your evaluation suggests you need a surgeon, I'll tell you.

### Why do my hips always feel tight even though I stretch?

Because tightness is usually your body protecting a hip that doesn't feel strong or controlled. Stretching gives short relief. Building strength through the full range is what makes the tightness go away.

### Is my hip pain actually coming from my back?

Sometimes. Pain in the buttock or down the leg can come from the low back, and the evaluation checks both. Often it's the other way around: hips that don't move well push extra load into the back.

### Why does the outside of my hip hurt when I lie on it?

That's often glute tendon irritation, sometimes called hip bursitis. It usually comes from how the hip is loaded on one leg, like stairs and running, and it responds well to the right strengthening.

### Can physical therapy help hip arthritis?

Yes, for many people. Strength and load tolerance around the hip can reduce pain and stiffness and help delay or avoid surgery.

### How long does it take?

Most people feel a difference in the first few weeks. A full plan is usually 6 to 12 weeks, depending on how long it's been going on and what you're getting back to.

### When should I see a doctor first?

If you can't put weight on your leg after a fall or injury, have sudden severe groin pain, or have hip pain with fever or pain at night that doesn't change with movement, see a physician first. Otherwise, you can start with physical therapy without a referral.
