# Physical therapy for shoulder pain

You've tried rest and the band exercises, and it still hasn't gone away. That usually isn't because your shoulder is damaged. It's because the shoulder has been doing a job the rest of your body isn't helping with.

### Pain reaching overhead

Shoulder impingement and rotator cuff tendinopathy that flares when you press, throw, serve or reach up to a shelf.

### Pain in the front of the shoulder

Biceps tendonitis (or tendinopathy) and that deep ache at the front of the shoulder that shows up with bench press, push-ups, dips or reaching behind you. If you've been told it's your bicep tendon, this is you.

### Rotator cuff tears

Partial and full tears, whether you're trying to avoid surgery or coming back after it.

### Pain at night

Shoulder pain that makes it hard to fall asleep or wakes you up when you roll onto it.

## Why your shoulder keeps hurting

Shoulders rarely fail on their own. The shoulder blade sits on your rib cage, so if your rib cage and mid back don't move well, the shoulder has to work harder to get your arm overhead. Sometimes the driver is even further away. A pelvis that sits unlevel changes how your whole trunk rotates, and the shoulder pays for it. Your body moves as a whole, so that's how we look at it.

## What happens in a shoulder evaluation

We look at the shoulder, but we don't stop there. We assess how your rib cage and mid back move, how your shoulder blade controls each reach, how your neck is compensating, and how your hips and pelvis set up everything above them. You leave knowing what's actually driving the pain and a clear plan to fix it.

## Why most shoulder rehab falls short

Most shoulder rehab is too specific. It's rotator cuff exercise after rotator cuff exercise with a light band, and not much else. Those exercises don't prepare you for real life: lifting your kid, carrying groceries, serving a tennis ball, pressing a barbell. Your shoulder needs progressive load to build real capacity, and the rest of your body needs to share the work.

## Two real stories

A tennis player came in with chronic shoulder pain. We got it feeling a lot better by working on the shoulder itself, but the big changes came when we looked at her hips. They weren't sitting level, and once we addressed that, her shoulder finally started to hold up.

A woman in her 40s had dealt with shoulder pain for years, and her neck hurt constantly too. Instead of more light band work, I taught her how to strength train and build real capacity. Her shoulder got stronger, and her neck pain went away. It had been overcompensating the whole time.

## Frequently asked questions

### Can physical therapy help a rotator cuff tear?

Often, yes. Many rotator cuff tears respond well to progressive strengthening, especially in active people. If your evaluation suggests you need a surgeon, I'll tell you.

### What causes pain in the front of my shoulder?

Front of the shoulder pain is often an irritated biceps tendon, and it usually shows up with pressing, push-ups, dips or reaching behind you. The tendon is often doing extra work because the rib cage, mid back or shoulder blade isn't. We calm it down, then fix what's overloading it.

### Why does my shoulder hurt at night?

Night pain is common when the shoulder is irritable. It usually eases as we calm it down and build its tolerance to load. We also screen for the less common causes that need a doctor.

### Why is my neck tight when it's my shoulder that hurts?

When the shoulder can't do its job, the neck and upper traps pick up the slack. Fix what the shoulder is missing and the neck usually settles too.

### Do I have to stop lifting or playing?

Usually not completely. We adjust what you're doing so you can keep training while the shoulder builds capacity.

### Do I need an MRI first?

No. Most people don't need imaging to start. If anything in your evaluation suggests you do, I'll tell you.

### Do you take insurance?

No. Revenant is cash-based and out-of-network, so every session is 60 minutes, one-on-one. Superbills are available on request.
