# Physical therapy for low back pain

![Human spine highlighted from neck to low back, low back pain physical therapy in Farmers Branch](https://www.revenantpt.com/hs-fs/hubfs/back-and-neck-pain.jpg?width=1520&name=back-and-neck-pain.jpg)

Maybe you're scared to bend over because the last time you did, your back went out. Maybe it's a constant, annoying ache you've learned to work around. Or maybe it's sharp, searing pain that shows up without warning. However it shows up, you've probably been told to rest, stretch and strengthen your core, and it keeps coming back anyway. Your back isn't weak or broken. It's been doing too much of the work for too long.

## Low back problems we treat

### Back pain that keeps going out

Flare-ups every few months from bending, lifting, sneezing, or sometimes nothing at all.

### Fear of bending and lifting

When you don't trust your back anymore, so you've stopped deadlifting, stopped picking things up off the floor, and move carefully all day.

### Constant stiffness and achiness

The low-grade tightness that limits how hard you train and how long you can sit, stand or drive.

### Disc bulges and herniations

Whether you've had an MRI that showed one or you've just been told "it's probably a disc."

### Sciatica and nerve pain

![Sciatic nerve path from the low back down the leg, sciatica physical therapy](https://www.revenantpt.com/hs-fs/hubfs/nerve-pain.jpg?width=1520&name=nerve-pain.jpg)

Sharp, burning or electric pain into the buttock or down the leg that's unpredictable and sometimes stops you in your tracks.

## Why your back keeps getting hurt

Your back is usually the hardest worker in your body. When your legs don't produce enough strength and drive, when you don't control your center of mass well as you move, or when you only have one way of moving, your low back picks up the slack. Over time it becomes a tired worker: always on, never getting a break, and the first thing to complain. It's not a hardware problem. It's a software problem. Your body has grooved one strategy, and the back is paying for it.

## Why rest, stretching and core work haven't fixed it

![Athlete setting up a deadlift, getting back to lifting after low back pain](https://www.revenantpt.com/hs-fs/hubfs/what-we-treat-hero.jpg?width=1520&name=what-we-treat-hero.jpg)

Most low back rehab is built on avoidance: don't bend, don't lift, brace your core, rest when it flares. Then comes a handout of bridges, clamshells and monster walks, the same one everyone gets, never progressed to match what your life actually asks of you. Massage, heat and adjustments can calm a flare, but they don't build capacity. Your back needs to be convinced it's safe to move, and the rest of your body needs to learn to share the load.

## What happens in a low back evaluation

We look at how you bend, squat, hinge, rotate and shift your weight, and how your hips, legs and mid back contribute to each one. Nothing is good or bad. They're strategies, and we find the ones that overload your back. You leave understanding why your back hurts, what's driving it, and a clear plan to build it back up.

## Real results

A man came in with back pain so debilitating he couldn't pick up his 2-year-old daughter. We got him back to full health and function.

Another man had dealt with recurring sciatica for years. We got rid of his mobility restrictions, and his symptoms went from limiting to barely noticeable.

## An honest note

Not everyone gets a perfect, pain-free fix, and anyone who promises you that isn't being straight with you. What you can get back is your life: lifting, training, playing with your kids, doing what you want without being scared of your back. And you'll understand how your body works, so you know how to keep it that way.

## Who this is for

Lifters and hybrid athletes, runners, golfers, parents, desk workers, and anyone whose back has started making the decisions. If you've stopped trusting your back, this is for you.

## Frequently asked questions

### Should I avoid bending and lifting?

No. Avoiding movement usually makes the fear and the stiffness worse. We start with what your back tolerates and build from there, so bending and lifting feel normal again.

### Can I deadlift again?

If deadlifting is one of your goals, for most people, yes. We work back to it with the right progression, so your hips and legs do the work instead of your back.

### Can physical therapy help a herniated disc?

In most cases, yes. Disc bulges and herniations show up on MRIs even in people with no pain, and most improve without surgery. If your evaluation suggests you need a surgeon, I'll tell you.

### Why does my back keep going out?

Usually because it's still doing the job your hips and legs should be doing. Until that changes, it stays close to its limit and small things tip it over.

### Can physical therapy help sciatica?

Often, yes. Most sciatica comes from an irritated nerve, not a damaged one. We calm it down, give it room to move, and build your tolerance back up.

### Do I need an MRI first?

No. Most people don't need imaging to start. If anything in your evaluation suggests you do, I'll tell you.

### Do you take insurance?

No. Revenant is cash-based and out-of-network, so every session is 60 minutes, one-on-one. Superbills are available on request.

Back not the only thing bothering you? See [everything we treat](https://www.revenantpt.com/what-we-treat?hsLang=en).
