The real reason

## Why your knee keeps hurting

The knee is a hinge caught between two joints that move in every direction. When the hip or the foot isn't doing its job, the knee picks up the slack.

### Something above or below isn't helping

Hips that don't control the leg, or a foot and ankle that don't absorb load, push extra stress into the knee. Sometimes it's an old ankle sprain from years ago.

### Capacity hasn't caught up

Mileage, weight or intensity went up faster than the tissue could adapt. It's not damage. It's a body that isn't ready for that load yet.

## Why most knee rehab falls short

Quad sets, clamshells and a bag of ice. Most knee rehab treats the knee as the whole problem and stops the moment it stops hurting.

That's not the finish line. If you squat heavy, run long or play a sport, your knee needs to handle that load again, and so do your hips and feet. We keep going until you're doing the thing you came in for, without thinking about your knee.

## Frequently asked questions

### Can physical therapy fix knee pain without surgery?

In most cases, yes. Most knee pain comes from how load is shared between the hip, knee and foot, not from damage that needs surgery. If your evaluation suggests you need a surgeon, I'll tell you.

### Should I stop running or squatting?

Usually not completely. We adjust how much and how hard while we build capacity, so you keep moving instead of losing fitness on the couch.

### Can physical therapy help arthritis or “bone on bone” knees?

Often, yes. Arthritis on an X-ray doesn't always match how much pain you have. Building strength and load tolerance around the knee helps many people move with less pain and delay or avoid surgery.

### Is my knee pain coming from my hip or foot?

Often. The knee sits between them and takes the load they aren't handling. That's why the evaluation looks at your hips, feet and ankles, not just your knee.

### Do you do rehab after knee surgery?

Yes. ACL reconstruction, meniscus surgery and knee replacement. We follow your surgeon's guidelines and build you all the way back to your sport or activity, not just to walking.

### How long does it take?

Most people feel a difference in the first few weeks. A full plan is usually 6 to 12 weeks, depending on how long it's been going on and what you're getting back to.

### When should I see a doctor first?

If your knee is hot, red and swollen without an injury, locks and won't straighten, or you can't put weight on it after an injury, see a physician first. Otherwise, you can start with physical therapy without a referral.
