# Is Out-of-Network Physical Therapy Worth It?

The short answer

Out of network physical therapy is worth it when you want more time, one clinician, and a plan built around a specific goal. You pay the clinic directly, can use HSA or FSA, and can submit a superbill to your insurance for possible reimbursement. If your copay is low and your goal is simple, in network may fit better.

Most of my discovery calls start with the same question. “Do you take insurance?”

The straight answer is no. Revenant is cash based and out of network, and I don't bill insurance.

But that's almost never the end of the conversation, because the real question underneath it is a fair one. Is this going to be worth it? You're not trying to spend money for the sake of it. You want to know what you're getting, how payment works, and whether it makes more sense than using the insurance you already pay for.

So let's walk through it honestly, including who I think should stick with in network care.

## Is out of network physical therapy worth it?

For most active people with a clear goal, yes, because what you're paying for is time, continuity, and a plan built around you. It's not a fancier version of the same visit. It's a different model of care.

A lot of insurance based clinics run on volume. Visits are short, the therapist is often managing several patients at once, and care gets split between different clinicians or assistants. Protocols fill the gap because there's no time for anything else. That's how an athlete ends up with the same exercises as an 80 year old with shoulder pain.

That's not a knock on those therapists. Most of them are smart and care a lot. The system they work in is the problem.

Continuity matters more than people expect. Research on patient satisfaction found that people treated by the same physical therapist for their whole course of care were 3.4 times more likely to be completely satisfied than people seen by multiple providers. Patients also tend to read rushed appointments as a lack of interest, and that hurts the quality of care.

Time also changes how well you understand your own body. In one study, 89% of patients were satisfied with how their physical therapist explained their condition, compared to 42% for their general medical provider. That explanation is a big part of the work. You can't stick with a plan you don't understand.

Then there's total cost, which is where most people do the math wrong. Don't judge by the price of one visit. Look at the whole picture: how many visits it takes, time off work, driving back and forth, and months of care that never actually fixed the problem. Research on individualized physical therapy and education found that people averaged 3.6 medical visits over the following year, compared to 13.2 with usual care, and total medical costs were cut by more than half. Thorough care up front tends to mean less care later.

### What's the difference between insurance PT and cash based PT?

| Cost structure | Copay and deductible each visit | You pay the clinic directly. Program cost is given at the end of the evaluation, before any commitment. HSA and FSA accepted |
| Paperwork | The clinic bills your insurance | You submit a superbill if you want to seek reimbursement |

## What is a superbill and how do I get reimbursed?

A superbill is a detailed receipt with your diagnosis and treatment codes that you can submit to your insurance for possible out of network reimbursement. I provide one on request, you submit it, and any reimbursement your plan pays goes directly to you.

Whether you get reimbursed, and how much, depends entirely on your plan. That's between you and your insurance company, so I can't guarantee anything. What I can do is make the process simple. Most plans accept claims through an online portal or by mail.

It's applied against your out of network deductible and coinsurance, and any reimbursement is paid to you.

## How do I find out what my insurance will cover?

Call the number on the back of your card and ask about out of network physical therapy benefits specifically. Five minutes on the phone will tell you more than any website.

### Questions to ask your insurance before you start

- Do I have out of network benefits for outpatient physical therapy?
- What is my out of network deductible, and how much have I met this year?
- Once my deductible is met, what percentage is reimbursed?
- Do I need a physician referral or pre authorization to submit out of network claims?
- How do I submit a superbill, online portal or mail?

## Who is out of network physical therapy not right for?

If your copay is low and your goal is simple, in network care may be the better fit, and I'll tell you that on a call. Not everyone needs what I do, and I'd rather you spend your money well than spend it with me.

In network probably makes more sense if:

- **Your copay is low and your goal is straightforward.** If you just need basic guidance and your plan covers it well, use it.
- **You've already hit your out of pocket maximum for the year.** Your insurance is carrying most of the cost at that point.
- **Paying out of pocket would put real strain on your budget.** Research shows financial pressure is a real barrier to sticking with care. A plan you can stay consistent with beats one you can't.
- **You need specialized hospital based care or equipment.** Some situations call for a multidisciplinary setting.

Out of network is also a poor fit if you're not ready to put in work. This model is built on coaching and collaboration. I'll give you the plan, the explanation, and the accountability, but the progress happens when you show up for it.

Where it fits best is the active person who has a specific goal, maybe already tried the standard route, and wants someone to actually figure out what's going on.

## Common questions

### Can I use my HSA or FSA for physical therapy?

Yes. Revenant accepts HSA and FSA cards, so you can pay for care directly from those accounts. If you have questions about your specific account rules, your plan administrator is the best person to ask.

### Do I need a referral to see a physical therapist in Texas?

No referral is needed to start at Revenant. If one becomes necessary later in your care, I coordinate it for you.

### When do I find out what my program costs?

At the end of your evaluation, before you commit to anything. You'll know what we found, what the plan is, and what it costs, and then you decide. The discovery call beforehand is free.

### Will my insurance definitely reimburse me?

It depends on your plan, so there's no guarantee. Asking your insurance the questions above before you start is the best way to know what to expect.
